Patient Notice
Your rights to your health information, and how to act on them. Texas law (House Bill 4224, Health & Safety Code §181.105) requires us to post step-by-step instructions for requesting your records, contacting our licensing authority, and filing a complaint.
This notice is provided to comply with Texas Health & Safety Code §181.105 and should be reviewed by legal counsel before launch. It supplements, and does not replace, our HIPAA Notice of Privacy Practices.
1. How to request your medical records
You have the right to request a copy of your health records held by Virtuosity Sports Chiropractic & Myofascial Therapy. To make a request:

Tell us you are requesting your medical records and include your full name, date of birth, approximate dates of service, the specific records you need, and where you would like them sent.
We will verify your identity and respond within the timeframes required by Texas law and HIPAA. A reasonable, cost-based fee may apply for copies, as permitted by law.
2. How to contact our licensing authority
Dr. Lindsay Hoffman, D.C.is licensed and regulated by the Texas Board of Chiropractic Examiners (TBCE). To contact the Board or ask about a provider's license:

3. How to file a consumer complaint
If you have a concern about your care or your health information, you may file a complaint with either of the following:

Go to the Board's complaint page at tbce.texas.gov/complaint-information.html.
Complete the complaint form online, or download it and submit it by mail, email, or fax using the contact details on that page.
Include dates, names, and a clear description of the issue so the Board can review your complaint.
Texas Attorney General — Consumer Protection Division
For complaints about the handling of your protected health information:
File online at texasattorneygeneral.gov/consumer-protection/file-consumer-complaint.
